Consult your practice nurse well in advance of your dates of travel. If travelling last minute still contact your nurse as it is better to have some protection than none at all.
When you contact the surgery explain to the receptionist that you need an appointment with the practice nurse for travel vaccinations. You will be asked to fill in a travel risk assessment form prior to your appointment.
Severnbank Surgery is a registered Yellow Fever vaccination centre and has been authorised by NaTHNaC (as the regulatory body for England, Wales and Northern Ireland), to administer yellow fever in accordance with International Health Regulations (2005). The surgery is able to administer Yellow Fever vaccinations to patients who are not registered as well as registered patients.
Please complete the Yellow Fever pre-vaccination checklist (PDF). Alternatively there are paper copies available from reception.
Yellow Fever fee of £81.08 includes the vaccination and the certificate.

Most commonly required travel vaccines are free, however there are charges for some less frequently needed e.g. Rabies vaccines.
For travel to a high risk Malaria country, medications are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because Malaria protection is not included in the services provided by the NHS and therefore is a chargeable service.

Yellow Fever risk assessment for unregistered patients
Please print, complete and deliver the form below to Severnbank Surgery along with a £24 non-refundable fee for risk assessment and admin (in addition to the fee for the vaccination which will be paid when vaccination is given). We will be unable to offer an appointment for a Yellow Fever Vaccine until we have received the completed risk assessment and the £24 fee. All information will be treated as confidential. Please check any medical details with your own surgery if in doubt.
